Be Well™

The Be Well™ mobile app simplifies the process for registered Be Well™ users and their pharmacy to connect. This allows patients and Caregivers (on behalf of their Person in Care) to easily:

• Access their medication profile

• Submit new prescriptions and refill requests online

• Check the status of their prescriptions on a mobile device

New prescriptions for Narcotic, Controlled and Targeted substances (NCTs) received through the Be Well™ app's PhotoRx feature cannot be processed without the actual prescription. Contact the patient if an NCT was submitted and inform them that the original prescription must be brought to the pharmacy before the prescription can be processed.

Eligibility Requirements

To be eligible to link their Be Well™ account, the patient or Caregiver must meet the following requirements:

1. Be 14 years of age or older

2. Be a retail patient

3. Not have any of the following patient statuses:

   • Inactive

   • Obsolete

   • Deceased

   • Pharmacy

   • Doctor's Office
