Patient Profile Report

The Patient Profile provides a detailed list of all prescriptions dispensed for a patient during the specified time period. This report can be used when prescribers request information about a patient's medication history.

  • Start Date and End Date - determines the date range for the report. Prescriptions are listed on the report if they have a Fill Date or Performed On Date that falls within the report period.

  • Patient - the report generates for the patient that is entered. This is a required field.

  • Doctor - the report lists prescriptions written by that doctor. When no doctor is entered, the report lists all of the patient's prescriptions that were dispensed during the report period, regardless of doctor.

  • English Only - the report prints instructions for each prescription in English, instead of the language set in Rx Detail.

  • Print Privacy Disclaimer - the report includes an English and French privacy disclaimer at the bottom.
