Frequency of Dispense

A Frequency of Dispense form or Frequency of Dispensing and Central Fill Authorization Form can be attached to a Patient Folder to indicate that a patient medication regiment requires a lesser quantity or days' supply than prescribed and the patient can be exempt from capitation rules.
