Patient Enrollment in diem health
This topic is part of the diem health section that is intended for independent pharmacies only. This topic is not applicable to Rexall pharmacies.
Once pharmacy consent has been provided for the diem health app, patients that want to participate in the diem health program can be enrolled in the Programs tab.
When the enrollment is complete, the patient has the ability to submit prescriptions for processing and have the ability to view all medications on file at your pharmacy through the diem health app.
A patient can only enroll in the diem health app program at one pharmacy. Once the patient enrolls, they can only submit prescriptions for processing at the pharmacy they are enrolled in.

To enroll a patient in the diem health app program:

Search for and select the patient.

Select the Programs tab.

Next to the diem health field, select Enroll/Decline.
The diem health Enrollment window appears.

In the Email field, enter the email address that the patient used to register on the diem health app.

Select Enroll. The diem health Enrollment window closes to the Programs tab. The status of Patient Enrollment Request Sent displays next to diem health.

The patient must accept the enrollment request in the diem health app for the connection to be completed.
Once the patient has accepted the enrollment request, the status of Patient Enrolled displays next to the diem health field.

If a patient no longer wants to be enrolled in the diem health program at your pharmacy, they can be removed from the program.
You must remove any linked patient prior to removing patients from the diem health program. For more information, see Unlinking patients in the diem health program.
To remove a patient from the diem health program:

Search for and select the patient.

Select the Programs tab.

Next to the diem health field, select Enroll/Decline.
The diem health Enrollment window appears.

Select Unenroll. A validation prompt appears.

Select Yes. The windows close to the Programs tab.
