Diabetes Education Tab

This topic is only applicable to Ontario pharmacies.

The Diabetes Education tab will only be visible when completing a Diabetes or a Diabetes Follow-Up medication review. This tab allows pharmacists to document general and specific details regarding a patient's diabetes management. All of the fields in this tab replicate the fields from the ministry's standardized Diabetes Education Checklist form.

Upon completion of the medication review, the information entered in this tab automatically populates in the Diabetes Education Checklist form and the Diabetes Education Patient Take-Home Summary form.

To complete the Diabetes Education tab:

  1. In the General Information section, enter information in the appropriate fields pertaining to the patient's diabetes management.

  2. In the General Diabetes Education section, select any checkboxes that apply to the patient. Additional fields will appear as you make your selections.

  3. In the Self-Monitoring Blood Glucose section, select all blood sugar management methods that apply. Additional fields will appear as you make your selections.

  4. In the Specialty Training section, select any training that was completed with the patient. Additional fields will appear as you make your selections.

  5. In the Resources Provided section, select any resources that were provided to the patient. Additional fields will appear as you make your selections.

  6. In the Referrals and Reason section, select the type of prescriber you have referred your patient to, if any.
